My Provider Support Services puts participants first in providing personalised NDIS supports across South East Queensland and the Fraser Coast area. Founded by two experienced previously Registered Nurses, our team delivers high-quality care with a focus on making your NDIS plan easier to understand and implement in the way you choose.

Getting started
We provide accessible services that are safe, engaging and responsive to your support and communication needs. We can also help you connect with an advocate or interpreter if needed.
Our services can be accessed through self-referral or third-party referral. You can contact us to assist you with the referral process.
The criteria to access our services are as follows:
β’ You hold a NDIS plan that identifies the services to be provided.
β’ The services listed in your NDIS plan aligns with the supports that we can provide as a NDIS registered provider.
β’ You have funds available in your NDIS plan to pay for services.

The first step in our process is to schedule an intake interview.
If required, we will provide you with information and support to access a person of your choice, such as an advocate, to assist you in this process.
This will allow us to establish the scope of support that you need and we are able to offer. We will contact you shortly after the intake interview to advise you of the outcome.

During the intake interview we’ll consider:
β’ Your choices and preferences.
β’ How your best interests can be maintained.
β’ The scope and urgency of your needs.
β’ The resources available within our team to meet your needs.
β’ How our services complement and interact with the other services you receive.

Once an agreement to proceed is reached, we will schedule a meeting with you to work with you to develop a Service Agreement that meets your needs and supports your choices.
This will include an assessment and planning interview and you will be able to negotiate any terms included in the Service Agreement.

The assessment and planning interview will include:
β’ Discussion on how we will provide our services to you, in support of your choices preferences, goals and support needs, and your preferred communication methods.
β’ Information about our policies and procedures, including our Feedback and Complaints Management Policy and Procedure and Incident Management Policy and Procedure.

At least once a year we will review the service agreement with you and your personal support network. Flexibility is provided in regard to the timing of review assessments based on your needs and wishes, and you can request a review at any time.
